Collection of Personal and Sensitive Information
For Allambee to provide services, we need to collect some personal information about you. This information will be collected from you directly or may come from a third party (e.g. a referral from your GP, other service provider).
Information collected may include specific information (e.g. demographic information, name, contact information) and case records (e.g. regarding the content of your services, reason for attending, treatment plans). Clinicians are required to maintain notes of their counselling sessions and treatment plans. Allambee may also collect de-identified information for the purposes of quality improvement, service evaluation and research.
Do I have to provide my personal information to Allambee Counselling?
Providing personal information to Allambee is voluntary, however if you don’t provide this information it may be difficult for us to provide the requested service.
How information is used & release of information?
Information will only be used for the purposes for which it was collected and will not be released or shared without your consent, unless in the following situations:
- If there is a perceived risk of harm and disclosure is necessary to lessen or prevent a serious threat to the life, health or safety of an individual or another
- Where there is a legal obligation to provide information (e.g. court subpoena, order to produce)
- With your consent as part of your engagement with our service (e.g. referral to another service, support letter, sharing of information between two services both working with you).
Is my information secure?
Allambee protects all information from unauthorised use, alteration, disclosure or destruction. Information both electronic and hard copies are stored securely and in the strictest of confidence. All reasonable administrative, technical and physical measures will be taken to safeguard your information.
Access to information is restricted to those individuals who need to know the information to process it and provide services. All staff members including volunteers are bound by strict confidentiality obligations
It is a requirement that we keep a record of your contact with our service. How long these records are kept depends on your age when you attended or contacted our service. If you were over 18 years of age, counselling records are kept for 10 years after the last contact and then securely and permanently destroyed. If you were under 18, your records will be kept until you turn 25 years of age.
Can I access my information or withdraw my consent for recording of information?
Yes. You can request access to the information collected about you or request that we correct or change information. Requests for access must be made in writing and proof of identity is required.
Will this policy be updated?
We may update this privacy statement from time to time and will make any new versions available on this site. We encourage you to periodically review this privacy statement for any changes.
